This mixed-method 4-year study will evaluate the implementation of a large-scale population-based colorectal cancer screening patient reminder and coordinated follow-up program.  This project will allow for the evaluation of the factors associated with implementation success across multiple levels: patient, primary care provider team, specialists, and other health plan staff and systems.

 

Evaluating a Preference-Tailored Intervention for Increasing Colorectal Cancer Screening

This study will integrate an innovative preference elicitation methodology into a decision tool to help patients clarify their preferences for CRC screening tests.  This study proposes and evaluates a conceptual framework where preference clarification leads to increased screening adherence through first improving informed decision-making, physician-patient communication, increasing knowledge and reducing decisional conflict.

 

Methods for Estimating the Incidence and Prevalence of Amyotrophic Lateral Sclerosis in the U.S. Population**

This project will use several population-based integrated health systems to gather data on the population denominator for estimating incidence and prevalence rates of ALS and other motor neuron disease overall, by socio-demographic subgroups and pay source.  This project also includes development of an infrastructure for collaborative research model using the data collected during this project, which will demonstrate the value of an ALS and other motor neuron disease registry and make salient contributions to our understanding of ALS and other motor neuron disease and its medical care burden.

 

Genetic Determinants of Inhaled Steroid Response in African-Americans with Asthma

This project will draw upon our experience and out diverse patient population to assemble a cohort of African American patients with asthma.  First, we will perform a genome wide association analysis which adjusts for racial admixture to minimize confounding by population stratification.  Secondly, we will use admixture mapping to identify genomic regions where a particular ancestry is overrepresented in ICS responders.  We will also exploit the benefits of our cohort design to examine genetic associations with quantitative traits related to baseline lung function.

 

The Impact of Diabetes Control on Employment and Work Productivity

A longitudinal study of patients with diabetes will examine the impact of diabetes control (glycemic, lipid, and blood pressure control) on labor market outcomes. Three research aims will be addressed:  1) determine the associations between diabetes control and the subsequent labor force participation of patients; 2) examine the impact of successful/poor management of diabetes on changes in work productivity and changes in wages and/or household income; and 3) estimate monetary value of productivity losses due to poor glycemic, lipid, and blood pressure control among patients with diabetes. 

 

Reanalyzing Data Sets using Path Analysis & Structured Equation Modeling to Reduce Racial/Ethnic Health Care Disparity

The purpose of this project is to re-analyze four existing data sets, using path analysis and structural equation modeling techniques, in order to estimate the effect of race/ethnicity on cancer treatment outcomes and intermediate outcomes (e.g., lipid control) in diabetes.  Racial/ethnic disparities have been identified in these data sets using regression analyses; we hypothesize that the effects of race/ethnicity are actually larger than those estimated using regression.  The path analysis techniques should produce a more accurate estimate of race/ethnicity effects.

 

Medical Care Burden of Cancer: System and Data Issues

The proposed project seeks to determine whether medical personnel with little or no previous training as ultasonographers can use transocular ultrasound to identify patients with elevated intracranial pressures.  The study will involve analysis of sensitivity, specificity, and positive predictive values of ultrasound measurements of optic nerve diameters when compared to a "gold standard" of concurrent measurements of intracranial pressures in an ICU setting.

 

The Pharmacogenomics of Inhaled Corticosteroid Responsiveness in Patients with Asthma

African American patients, in particular, appear less likely to respond to corticosteroid therapy when compared with white patients.  However, it is not currently known whether this difference results from genetic or environmental factors, or whether differences exist in inhaled steroid responsiveness (i.e., the recommended route of therapy).  This question is of particular importance, since African American patients suffer disproportionately from asthma-related complications.  In this proposal we plan to assess inhaled corticosteroid responsiveness by measuring change in FEV1 among both African American and white patients with asthma following 6 weeks of inhaled beclomethasone diproprionate treatment.  This study group will also serve to identify genetic loci associated with ICS responsiveness.  Participant DNA will be genotyped using genome-wide SNP arrays. The subsequent association analyses will use common and novel techniques to account for underlying population structure, such as genomic control, principle components analysis, and admixture mapping.

This project addresses recent Institute of Medicine calls for increased research on the reentry transition in cancer survivors, particularly among understudied cancers like colorectal.  The project will assist colorectal cancer survivors in their reentry transition from active treatment to the early stages of cancer survivorship through telephone-based education and coping skills interventions that will inform and empower colorectal cancer survivors to self-manage their stressors in conjunction with their healthcare providers, family and community, and workplace.  The project will also explore the economic impact of the intervention, including estimation of program costs, medical care utilization, and employment and productivity.
