of renal impairment in actuate heart failure patients admitted
into hospital, examine the frequency and magnitude of worsening
renal function in acute heart failure patients in hospital and
the relationship between worsening renal function and treatment
pattern in the hospital. It will also determine whether
worsening renal function in patients with acute health failure
is associated with greater in-hospital resource utilization
including length of stay, likelihood of an ICU stay, and time of
ICU stay and determine whether worsening renal function in
patients with acute heart failure is associated with increased
hospitalization or death.

PROPOSALS SUBMITTED (PENDING OR /NOT FUNDED)
Pharmacogenomics of Inhaled Corticosteroid Responsiveness in
Patients with Asthma
P.I.:
Keoki Williams, MD, MPH
Funding
Source: NIH
Funding
Amount: $3,634,668
Submitted: 10/5/07; Resubmitted 7/5/08
Status:
Pending Award
African
American patients, in particular, appear less likely to respond
to corticosteroid therapy when compared with white patients.
However, it is not currently known whether this difference
results from genetic or environmental factors, or whether
differences exist in inhaled steroid responsiveness (i.e., the
recommended route of therapy). This question is of particular
importance, since African American patients suffer
disproportionately from asthma-related complications. In this
proposal we plan to assess inhaled corticosteroid responsiveness
by measuring change in FEV1 among both African American and
white patients with asthma following 6 weeks of inhaled
beclomethasone diproprionate treatment. This study group will
also serve to identify genetic loci associated with ICS
responsiveness. Participant DNA will be genotyped using
genome-wide SNP arrays. The subsequent association analyses will
use common and novel techniques to account for underlying
population structure, such as genomic control, principle
components analysis, and admixture mapping.
Lung/Colon
Cancer Outcomes – Cancer Research Network (Years 9-11)
P.I.:
David Harrington, MD
Site
P.I.: David Nerenz, Ph.D.
Funding
Source: NCI
Funding
Amount: $193,697
Submitted: 11/25/08
Status: Pending
The
overall objectives for the CanCORS renewal include: an
understanding of the longer-term health outcomes for patients
enrolled in the two CanCORS cohorts, approximately 5-7 years
post-diagnosis. This additional analysis is necessary to fully
characterize the impact of initial cancer care (that was
evaluated in the previous funding cycle). In addition,
disparities in health outcomes and factors that are likely to
mediate such disparities will be evaluated. Additionally, an
assessment of the quality of follow-up care delivered to
patients beyond the initial acute treatment phase will be
completed.

The
application is a response to an RFA to establish Clinical and
Translational Science Award (CTSA) programs and centers. The
grant is fundamentally an infrastructure grant to create a major
overhaul of the support infrastructure for clinical and
translational research. The WSU-HFHS application is a
collaborative effort involving multiple faculty members or
investigators from both institutions. Dr. Nerenz served as one
of the five members of the application's Executive Committee and
as the leader of the Translating Research Into Practice (TRIP)
section of the application.
Time
Allocated to Mental Health in Primary Care Visits
P.I.:
Ming Tai-Seale, MD (Texas A&M)
Site
P.I.: Jennifer Elston Lafata, Ph.D.
Funding
Source: NIH
HFHS
Funding Amount: $504,104 (total cost)
Status:
Not Funded
The
main goal is to conduct a comprehensive analysis of the factors
influencing primary care physicians’ allocation of clinical time
and how well that time is used to assess and treat older adults
with mental illnesses. The long-term objectives of this project
are to inform policy and clinical practice so that more
efficient and equitable mechanisms of financing and organization
of mental health services in primary care can be developed.
Surveillance Care Decision-Making among Colorectal Cancer
Survivors
P.I.:
Jennifer Elston Lafata, Ph.D.
Funding
Source: NCI
Funding
Amount: $2,165,641(total cost)
Submitted: 6/06; Resubmitted: 3/19/08 (not funded – score 190);
Resubmitted: 11/08
Status:
Not Funded
The
overall objective of this proposal is twofold. First, we will
describe the quality of the surveillance care decision-making
process among colorectal cancer survivors, both overall and by
patient-defined subgroups. Second, we will examine the extent to
which variation in surveillance care use is a result of an
informed decision-making process and reflective of patient
preferences.
Measuring
and Reporting Health Care Outcomes in Vulnerable Populations:
Should We Adjust for Sociodemographic Characteristics?
PI:
David Nerenz, Ph.D.
Funding
Source: Robert Wood Johnson
Funding
Amount: $310,000 (total cost)
Submitted: 7/25/08
Status:
Not Funded
The
overall goal of the project is to identify, compare, and
evaluate alternative ways to take sociodemographic factors
(e.g., race/ethnicity, SES, primary language, literacy level)
into account
when
measuring, analyzing, or publicly reporting health care outcome
data.
